CityChurch provides Gap coverage to help offset some of the medical expenses. If you have had any of the events below, download the form and submit for reimbursement. You can fax the form. The phone number is on the form.

 

• In hospital stays of 18 or more hours........$1,500 reimbursement

• Outpatient surgery.....................................$400

• Imaging......................................................$100

• ER visit.......................................................$50

• Physician office visits……………………….$25 (up to 5 visits per family per year)
